Clinical and morphological comparison of respiratory failure in patients with COVID-19 who died under different options of respiratory support

The aim. Based on the study of effect invasive mechanical ventilation and NIV in CPAP mode pathomorphosis lung damage patients with HRF caused by SARS-nCoV-2 deaths intensive care unit (ICU), determine safest method respiratory support.
 Materials methods. included morphological material from 20 (COVID-19) who died ICU. Group 1 received non-invasive through a face mask (n=10), group 2 - underwent (n=10). prepared sections, 5 μm thick, were stained according to Van Gizon method. Photomicrographs taken using Zeiss ZENliteimaging. Data are presented as M [25-75] P±Sp. Statistical analysis results was performed program “Statistica 10”. Significance differences indicators assessed nonparametric Wilcoxon test, parametric Student's test. considered reliable at values p<0.05.
 Results structure lungs corresponded exudative phase DAD severe edematous-hemorrhagic syndrome, signs interstitial pneumonia desquamation alveolocytes formation hyaline membranes. In tissue there picture proliferative pneumonia, development focal fibrosing alveolitis. Thus, ventilation, can accelerate irreversible processes form alveolitis promote ventilator-associated pneumonia
